For the first question, the answer is MANY for spam posters, SOME for naughty-never-want-to-listen-to-moderator's-advice-to-edit posters (this is usually for a very short period) and a FEW doing self-banning for too-proud-to-follow-site-guidelines poster. All of the reason are self-explanatory,

For the second question, the answer is we never ban the books but we listen to the authors' notes by respecting their wishes.
