So I see a hell of a lot of really poor PDF conversions being posted as epub or mobi files and this is entirely unnecessary. There are certain programs that make PDF conversion relatively easy.

First off image based PDFs are not generally convertible. They have to be OCR-ed with Abbyy FineReader or similar software.

Secondly, a text-based PDF usually has page-numbers and split paragraphs that will ruin any conversion. Use Mobipocket Creator to import the PDF and build a PRC. file. Import this file into Calibre and convert to epub. Use Sigil to edit the epub, create a Table of Contents and remove any remaining errors. Then convert to mobi for Kindles using Calibre. It's really quite easy and produces excellent results. Not as good as a retail epub or mobi but good enough. There are many books that are available to buy as epubs but only the PDF versions are being shared. Many of these PDFs are text-based and easily converted using the above method. I hope this helps someone produce better conversions.
