Hi. This is a YA book that I've read years ago. The heroine is just a high school grad and had a heart transplant. She went to buy an urn for her (old) heart and plan to travel and throw some of the ashes. Now this hero saw her when she bought the urn and decided to travel with her. Until one day during one of their stops, the girl is having some emergency problems of her heart transplant or something. When they got home, the guy is having this guilt that because of him the girl got those problems. And they went to college separately and met again. I think the guy visits the girls school & surprises her.
