No , the original format must not be changed
you choose the type of advertisement then write it as the original form:
"this app has (banner, interstitial, push,........) advertisement"
if no advertisements:
"this app has no advertisements"

Advertisements included in the app. Use the exact text used in the template but replace the part in capital letters (CAPS). If your release has no ads you do not need to specify any ads, if your app does have ads you need to specifically state which ones (saying This app has advertisements will not be enough).
