It says it can send SMS to premium number

But the app itself don't have permission to access the sms, how can it send anyway?

It's difficult to say why it's detecting the APK like that. My guess is the different signature. Feel free to decompile the APK to compare it to the original, and you'll see that I've done nothing malicious.

At this point, the only way for me to [possibly] fix that is to switch keys again, which I don't plan on doing, since people still rely on the Alternate Signature version. I'd hate to post three differently-signed versions everytime.

No problem for me as I am still using the app on my phone and tablet, and the permissions look rather benign.

I tried sending both versions via Gmail with the same result, so it may not be the signature.
I am thinking that it may be a false positive from the 3/53 report. Possibly Google now route/scan Gmail with the VirusScan(Google owned now) engine.

seeing there are no viruses on android, bugs ya but thats about it. So no doubt.. a false positive like everything else.
