About "Draw over other apps" permission: This permission is required to block system buttons when input blocking is activated on the playback screen.
About "Pair with Bluetooth devices" permission: This permission is required to improve AV sync when Bluetooth headset is connected.

Per Balatan "Ultra Mod have a few useless features removed and a deep resources (png, jpeg, json, etc) optimized but not compressed, that`s the reason of the same size. Like you know the new Android OS like 12 or 13 and so on, do not support compressed resources and app wont install on such devices"

Search "mx player custom codec xda". Then click on the GitHub link given on that XDA thread and download the version that suits your device from the releases section. (If you don`t know which one, then download the mx_aio (All in one) one).
