Er, pardon my ignorance, but what does this do? Unlock apps, or prevent trial apps from accessing Market making them usable?? Can I patch all the apps, or is there are requirement?

The patcher scans by himself all apps. If one app is crackable, then the app shows up with green text. It depends on luck (see name), to crack the app fully. If an app is trial only and/or it has a custom license - the patcher will not be able to crack them. Only apps that verify the license with Google are possible crackable by Lucky Patcher.
