Again, I've experienced this problem with all R3 versions using both PDA and PNA with 2008Q2 North America maps (Pennsylvania to be specific).
It would be great if someone can confirm this now that I have more specifics as to EXACTLY when/how it happens!

I really come to the conclusion, that this is a map problem...
I wake up from stand-by daily in an off-road parking-lot (although it's only a few meters next to the road, but it's showing as off-road). When I then drive onto the road, it's recalculating and operating normally - with German TA maps (did not try with the NT ones).
